The ICAM Lead Investigator course is provided in association with Edith Cowan University.

In this course, we will guide you through our incident investigator's toolbox, the ICAM analysis process, and how to communicate the investigation findings.

This course is ideal for employees who will form part of an incident investigation team, including managers and supervisors. It provides the underpinning knowledge and practical skills needed to undertake and lead an effective investigation.

The main objective of an investigation is to prevent the incident from happening again. The way the investigation is carried out depends on the seriousness and complexity of the incident.

Findings and recommendations must be based on conclusions drawn from the information and evidence collected during the investigation. For the process to be successful, it is essential to establish the events that led up to the incident, the facts of the incident itself, and what occurred immediately afterward.

Upon completion of the course, participants will be able to;

  • Collect evidence

  • Evaluate information

  • Identify unsafe conditions, acts or procedures

  • Identify deficiencies in systems that permitted the incident to occur
