Our Approach

All investigations are planned and executed using a structured, evidence-based methodology consistent with:

  • The Work Health and Safety Act 2020 (WA)

  • The Australian Government Investigations Standard 2022

  • Established public sector investigation standards

For WHS investigations, we apply recognised methodologies such as ICAM (Incident Cause Analysis Method) and RCA (Root Cause Analysis) to ensure contributing factors are thoroughly explored and risk controls are meaningful and effective. All findings are clearly documented to support legal defensibility and practical safety improvements.

We also apply a trauma-informed approach where appropriate, particularly in psychosocial or sensitive investigations. This includes:

  • Establishing trust and safety in the interview process

  • Being transparent about process, purpose, and confidentiality

  • Using open, non-judgemental questioning and active listening

  • Minimising the risk of re-traumatisation for participants

Investigations are guided by an understanding of legal principles relevant to WHS and compliance.
