For WHS investigations, we apply recognised methodologies such as ICAM (Incident Cause Analysis Method) and RCA (Root Cause Analysis) to ensure contributing factors are thoroughly explored and risk controls are meaningful and effective. All findings are clearly documented to support legal defensibility and practical safety improvements.
We also apply a trauma-informed approach where appropriate, particularly in psychosocial or sensitive investigations. This includes:
